2007/2008 Yearly Forms
These forms must be submitted new each school year.
Put all forms for your family in a single envelope and return to a homeroom teacher by the first day of school.

- Required - One Per Student
- - Clinic Information/Emergency Form
- - Diocesan Technology Acceptable Use Form : (Return page 3 for Middle School students and return pages 4 and 5 for students in Grades K-5)
- Required - One Per Family
- - Photo Release
- - After Care Release Form
- - Family Demographics (only for families new to CCS)
- Special Use Forms - Complete as needed for your circumstances
- - Driver Volunteer Form (must be received for any family member who may drive for field trips)
- - Pick-Up Authorization (to give someone else permission to pick up your children)
- - Prescription or Over the Counter Medication Release Forms (one per student who needs medication). The appropriate forms must accompany medications before they can be administered.
